Lambeth is changing the way care is delivered by offering more services in the community – closer to home and outside of hospital settings. The aim is to develop neighbourhood teams that bring together professionals to help people stay healthy, live more independently and avoid unnecessary hospital visits.
At first, neighbourhood teams will focus on:
· Children and young people with complex needs
· People with multiple long-term conditions
· People who are frail
· People who need mental health support
