We regularly receive requests from patients for letters written by a GP writing in support of their local authority housing application or appeal.
Minet Green Health Practice is unable to provide bespoke GP letters regarding housing. Should patients contact us to request a housing letter we will provide you with, for free:
- a summary of your medical record formatted as a letter addressed to your housing officer
- a consent form for you to complete and send to your houisng provider, authorising them to contact us on your behalf should they require more information
- a letter explaining that we are no longer providing bespoke GP letters for housing
