Under the Data Protection Act 2018 you do not have to give a reason for applying for access to your health records.
Your health record includes information like any conditions or allergies you have and any medicine you’re taking.
Most patients will automatically have access to more information added to their health record. This includes letters, test results and appointment notes.
You can access your health record and nominate someone you trust to access it too.
You can use your NHS account (through the NHS website or NHS App) or NHS App to access your health record.
Request access to view your full health record online
First, you need to register for online services and prove who you are. To request access:
- use your NHS account (through the NHS website or NHS App)
- contact us via Accurx and ask us to set up online access for you
You’ll need to contact the practice for online access to your full health record. By default you will only have limited access.
Request a copy of your health record
If you would like to request a digital or hard copy of your medical records, please contact us via Accurx and ask us to send you a copy of the “Access to Medical Records” form.
We will normally provide these to you by email or via the NHS App. For repeat requests which you require in printed form, you may be charged a small fee for materials and administration costs.
Access to other medical records
For any other medical records, you need to ask for them from the NHS service you were seen at. This includes your hospital records.
