To update details like your name, address, phone numbers or email address:
- use your NHS account (through the NHS website or NHS App)
- use the Accurx service to request the “Change of Personal Details” form

If you have moved to a new address, you will need to be in our catchment area to stay registered with our practice.
Changing your contact details
We may need to ask for some evidence in order to update certain information.
Changing your name
You will need to show us proof that you have changed your name. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
