Medical reports

We receive many requests to provide reports and copies of medical records to third parties, including insurance companies, solicitors, the Department for Work and Pensions (DWP), Transport for London (TfL), and the Driver and Vehicle Licensing Agency (DVLA).

If you are a representative from a third party organisation and would like to request a medical report for one of our patients, please contact us via email. If you do not have our email address please call us on 020 3930 3533 to request it.

If you are a patient, please contact us via Accurx with full details of your request and our team will get back to you as soon as possible.

The NHS does not fund letters and medical reports so in most cases the practice will charge a small fee to cover the cost of producing the report. You can see the current price list of fees for private services here.

Access to your medical record is your legal right and we will always be happy to arrange this for you, free of charge.

I need a letter from my GP

Our doctors are frequently asked to provide private or ‘To Whom it May Concern’ letters or reports for third parties. The standard fee for letters without medical opinion is £25 and the fee for letters with medical opinion is £50, which is payable in advance.

Why do I have to pay?

The preparation of these letters or reports is not covered by the NHS. The GP Partners must complete them during their own time and as such there is a fee payable. All letters require the doctor to take time to review a patient’s medical records to ensure that the information provided is correct, then the doctor has to type and print it.

There is some further information on this subject on the website of the British Medical Association (BMA).

How long will it take?

Private letters do not take priority over NHS work and hence may take some time to prepare. We aim to produce letters within 28 days but this may take longer during especially busy periods.

But I need it today/tomorrow/next week/sooner than that

In genuinely urgent cases, we will do our best to meet your deadline; however, we cannot promise this. If you cannot wait we may refund your fee, at our discretion, if the letter has not yet been written. If you have a specific deadline please make this clear in your request and submit it to us at least one week before the deadline.

The doctor has to write these exact words…

We are very sorry, but we usually cannot accept requests for the doctor to write the letter in a specific way. The doctor will write what they know is supported by your medical record, and their opinion on what that should be is final. Please note that our doctors reserve the right to refuse your request or provide alternative wording.

Please note: if you insist on the doctor changing your letter to suit what you think it should say, or if you want to dictate the contents, we will withdraw our offer to write the letter and refund your fee. If the letter has already been written we cannot provide you with a refund. If the letter has not yet been written, we may agree to refund your fee at our discretion. Please remember that the doctor is not obliged to write a letter for you at all, as this is not an NHS requirement.

I need a letter from my GP to support a housing issue

We are unfortunately rarely able to accomodate requests for letters regarding housing. The council or housing association will contact us directly should they require more information, and if they have your consent to do so.

Examples of suitable requests:

  • Fitness to travel, or cancellation of holidays
  • Confirmation of medication or a medical condition
  • Letters for school, universities, employers or courts, etc.

Example of requests we cannot fulfil:

  • Confirmation of address or other proof of residence or ID (We have no means of verifying this)
  • Countersigning passport applications
  • Letters stating something that is not supported by your medical record
  • Completion of Blue Badge application forms (see below for details)
  • Completion of Power of Attorney applications (see below for details)

If you understand and agree to the above terms and still want the GP to write a private letter, please contact us via Accurx and ask us to send you a copy of the “Request a letter from your GP” form.

Blue Badge applications

Unfortunately the practice is unable to complete Blue Badge medical information forms. This includes the “non visible disability” form and others of this nature.

We are able to provide a copy of your medical record and a covering letter explaining our reasons for not completing these forms, which you can present to the Blue Badge administration team. We are aware that our patients have been successful in their Blue Badge applications following submission of one of these medical records with covering letter. This service is free of charge.

Alternatively, on request we can produce a “To who it may concern” letter confirming diagnosis which patients can use to support their Blue Badge application, but there will be a charge for this.

Power of Attorney

Please note we are unable to complete forms or provide opinions with regard to Lasting Power of Attorney for Property & Finance, or Lasting Power of Attorney for Health.
